It may be a virus that will attack your computer.
The file may not be useful also.
If there were such files or medicine available, it would usually be announced by the government so that people can buy thoe products to prevent themselves from getting the virus.
John should not open the file as it is possible that the email is just spam or a virus. He should check with reliable sources such as doctors or his parents before buying the medcine. You never know if it is illegal drugs or whatever.
